Now this digest has grouped some messages on a topic together, yet others with the same subject are not grouped. Is the problem the way users are posting messages? It seems like a small thing, but it's driving me nuts.
Email list usually are not thread based, unless your client combines emails with the same subject into thread-like groupings. I think Nabble does some threading though, but I do not use Nabble often.
Yes, I have seen many lists/forums that are threaded, some good and some bad.
You are talking about "digest" lists. What are your reasons for using the digest list? I personally do not use one, so I do not know what the issues you have.

It is threaded and you can get an overview of the threads written to in the last week or however long. (including resurrected threads). It's not "everyone's cuppa tea" but i like it and i increasingly find it useful.

Nearly but not quite. "Many" implies a lot but does not mean a majority. It could be that 99% of mailing lists don't thread but the remaining 1% might be 100 which still easily qualifies as "many".
I like the troll counting system in Terry Pratchett books. It's a bit like binary but never quite reaches 10 eg 1, 2, many, lots.

Unlike other mailing list managers (e.g. mailman) you cannot reply to
messages directly in the digest mode by copying the original message
subject and retain the message thread.
Browser based lists/forums, not email based lists.
Nabble is browser based, while I use the email based list for LO.
I do not remember ever seeing a email based list that worked, and a good threading/grouping system for any of the email clients I have used.

Maybe something is wrong with the headers, or the digest only groups
directly related messages together (maybe B is only grouped with A if it
is a reply to A).

E-mails *are* usually threaded, apart from messages sent with a couple
bad clients.
Even if you've never seen a client threading e-mails, these e-mails
still have the threading information in the headers.
Don't confuse this with subject-based grouping of messages.
